Subject

Implementation of FMEA (Failure Mode and Effects Analysis) in the process of evaluation of suppliers

Short Description of Subject:

FMEA is a tool for identification and analysis of possible (or specific pre-determined) scenarios of failure, the ways how they could manifest (failure effects) and what are the risks connected with the process i.e. the product.
FMEA enables in a systematic manner for such identified scenarios of failure mode and effects to establish actions in order to eliminate them completely, or to significantly reduce possible risks.

As a tool it is used most often in the phase of analyzing the program that are led according to DMAIC (LSS) methodology FMEA may be successfully used when needed as part of routine process. An example of that is process of evaluation of suppliers. Besides short theoretical presentation of the FMEA tool, one example from the practical experience is presented, in which in order to identify potentially risky phases of the production process evaluation of production process of the supplier for packaging was evaluated.
